All medical providers are expected to follow Medical Advisory Board (MAB) protocols and bill according to the Rhode Island Fee Schedule.

Please complete your Department of Labor & Training and Medical Advisory Board forms and fax to Beacon Mutual as soon as possible at 401-825-2980.

As a Medical Provider, you can expect:

  1. Prompt payment for services rendered.
  2. Professional response to requests and/or questions.
  3. Personal communication. (Options: telephone or voice mail, e-mail, facsimile).

Providers should call the Provider Call Center at 1-401-274-4848 when looking for the status regarding payment of a bill.

Providers should call 1-401-459-1664 when looking for authorization to treat an injured worker.
